Insurance Script

Cared For Nutrition does not bill insurance directly, however we will provide a Superbill for you to submit to your insurance for reimbursement. Below is a script to use when contacting your insurance company.

Step 1: Have your insurance card ready and call the member services line listed on it (you can also find this through your insurance’s online portal if you have an account).

Step 2: Ask the following questions:

  • Does my plan cover out-of-network outpatient nutrition counseling? (the billing codes are 97802 and 97803) If yes, how many sessions are allowed?

  • Are nutrition services covered under code ICD-Z71.3, or are only certain diagnoses covered? Which ICD-10 diagnoses are covered for nutrition services? (write these down)

  • Do you need a physician referral?*

  • What is your process for reimbursing me?

  • Take down the agent’s name and reference number. You will need these to dispute any rejected claims.

*If your insurance pays for a specific diagnosis, you must provide a referral with this diagnosis so that I can add it to your Superbill. It is out of my scope as a dietitian to diagnose you, therefore you must get a diagnosis from a qualified healthcare professional.
